Maria's Heart

Máriino srdce – slovenská verzia pozri nižšie

During the past year we have learned that Maria has Rheumatic Heart Disease. She was heaving very high pulse, chest pain, shortness of breath, and fatigue. Seeing doctors, she was referred to a cardiology specialist. Having done tests and ultrasounds ( TEE - Transesophageal Echocardiogram Procedure ) 2 times,  It has shown progression to the point that she needs to have surgery - Mitral valve replacement and aortic valve replacement with a mechanical valve, pulmonary vein isolation, left atrial appendage exclusion. The surgery was scheduled, however there were different outcomes of initial tests that were performed immediately prior to the cutting procedure, as a part of standard procedure. The results were not what they anticipated, the function and the performance of her heart seemed to be in better shape as what the previous tests displayed. The Team that was caring for her, performed additional tests to see if the initial findings can be replicated ( blood regurgitation, amount of blood passing through at different heart rates at diferent blood pressure levels) They were not able to replicate the data.

Upon couple of conversations it was decided that it would be better not to replace the valves at this time - postpone the surgery and continue to monitor the progress. ( feel free to read up on more datails and timing of events bellow.

Maria, Rasty and the kids are very greatefull for the outporing support, prayers and help from family and friends all over the world during this time. We know we have a loving God who is in control of our lives and we trust that he has a purpose in all of this that we are going through. We trust in his care for us and trust in His timing for when the surgery needs to be done.

Next steps

We will be following up with the cardiologists, both her regular and the surgeon to come up with a treatment plan and also to learn how to live until the surgery is absolutely necessary.  

Surgery Day 1/30/2025

3PM:

Maria was discharged. The Surgeon came and talked to Maria and me and explained things very well to us as to why the decision was made not to proceed with the surgery at this time. He explained the next steps and things we should plan for going forward, follow-ups, medication adjustment, etc. Even though we had different plans for today, we do believe that God has a reason for allowing things to go this way. We are very grateful to family and friends that were, and are praying for us, helping and being ready to help in different ways. It is an amazing feeling to be surrounded by such people in our lives. 

12:00 PM

To sum up the morning, based on the findings which showed better results than it was before ( tests that suggested we go with the surgery)  the surgeon feels that the benefits of postponing surgery outweigh the risks of proceeding with the surgery today.  Maria is slowly waking up - confused as to what happened or did not happen. 

We are very grateful for all the outpouring support and prayers of people around the world ( MN, WA, CT, FL, Norway, Slovakia, Australia)  and who knows where else. Feeling God's hand in all of  this. 

 

10:20 AM

After stressing heart even more the heart function is still within same results as it would be with new valves.  I have agreed with the surgeon not to proceed  based on findings. They will wake her up and we will also discuss with her.  The secondary reason is also delay the other risks that would come up after the surgury with the new valves etc. 

 

9:50 AM update

Talked with the surgeon, after their initial ultrasound, and some tests.  They put the heart to stress test to increase heart rate and pressure, they noticed that at 80bpm and higher blood pressure there is not much difference in the blood flow  that the new valves would provide benefits. 

He feels that the current valves would do the same job for at least 5 more years. They will increase the stress test to even higher heart rate to assess the situation.  The surgeon wants to make sure that he can for sure make a difference for her life after the surgery.  With his initial findings he doesn't feel that way. So far they have not started the actual surgery.

Počas minulého roka sme sa dozvedeli, že Mária má reumatickú chorobu srdca.  Mala veľmi vysoký pulz, bolesť na hrudníku, dýchavičnosť a únavu. Po návšteve lekárov ju poslali ku kardiologickému špecialistovi. Po 2-krát vykonaných testoch a ultrazvukoch ( TEE - Transezofageálny echokardiografický postup ) sa ukázalo progresie do tej miery, že potrebuje operáciu - náhrada mitrálnej chlopne a náhrada aortálnej chlopne mechanickou chlopňou, izolácia pľúcnej žily, vylúčenie prívesku ľavej predsiene. Operácia bola naplánovaná, avšak výsledky úvodných testov, ktoré boli vykonané bezprostredne pred nárezom v rámci štandardného postupu, boli odlišné. Výsledky neboli také, aké očakávali, funkcia a výkon jej srdca sa zdali byť v lepšom stave, ako ukázali predchádzajúce testy. Tím, ktorý sa o ňu staral, vykonal ďalšie testy, aby zistil, či je možné zopakovať počiatočné nálezy (krvná regurgitácia, množstvo krvi prechádzajúcej pri rôznych srdcových frekvenciách pri rôznych hladinách krvného tlaku). Údaje neboli schopné replikovať.

 

Po rozhovoroch sa rozhodlo, že v tejto chvíli bude lepšie nevymieňať chlopne - odložiť operáciu a pokračovať v sledovaní pokroku.

Mária, Rasťo a deti sú veľmi vďační za podporu, modlitby a pomoc od rodiny a priateľov z celého sveta počas tohto obdobia. Vieme, že máme milujúceho Boha, ktorý má kontrolu nad našimi životmi, a veríme, že má v tom všetkom, čím prechádzame, nejaký zámer.  Veríme v jeho starostlivosť o nás

Den Operacie - 30.1.2025 

15:00

Mária bola prepustená z nemocnice. Chirurg prišiel, porozprával sa s Máriou a so mnou a veľmi dobre nám vysvetlil veci, prečo padlo rozhodnutie v tejto chvíli nepokračovať v operácii. Vysvetlil nám ďalšie kroky a veci, ktoré by sme si mali naplánovať napredovanie, sledovanie, úpravu liekov atď. Aj keď sme dnes mali iné plány, veríme, že Pan Boh má dôvod dovoliť veciam ísť týmto smerom. Sme veľmi vďační rodine a priateľom, ktorí boli a modlia sa za nás, pomáhajú a sú pripravení pomôcť rôznymi spôsobmi. Je úžasný pocit byť obklopený takýmito ľuďmi v našich životoch.

Aby som zhrnul naše ráno, na základe zistení, ktoré ukázali lepšie výsledky ako predtým (testy, ktoré navrhli ísť na operáciu), chirurg má pocit, že výhody odloženia operácie prevažujú nad rizikami dnešného chirurgického zákroku. Mária sa pomaly prebúdza – zmätená, čo sa stalo alebo nestalo. 

Sme veľmi vďační za všetku podporu a modlitby ľudí na celom svete (MN, WA, CT, FL, Nórsko, Slovensko, Austrália) a ktovie kde ešte. Cítime v tom všetkom Božiu ruku.

Po ešte väčšom zaťažení srdca je funkcia srdca stále s rovnakými výsledkami ako s novými chlopňami. Súhlasím  s chirurgom, že nebudem pokračovať na základe nálezov. Zobudia ju a budeme s ňou aj diskutovať. Sekundárnym dôvodom je aj oneskorenie ďalších rizík, ktoré by nastali po operácii s novými chlopňami atď

Hovoril som s chirurgom po počiatočnom ultrazvuku a niektorých testoch. Vykonali záťažový test srdca, aby zvýšili srdcovú frekvenciu a tlak, všimli si, že pri 80 bpm a vyššom krvnom tlaku nie je veľký rozdiel v prietoku krvi, ktorý by nové chlopne poskytovali. Má pocit, že súčasné ventily by rovnakú prácu robili ešte minimálne 5 rokov. Na posúdenie situácie zvýšia záťažový test na ešte vyššiu srdcovú frekvenciu. Chirurg sa chce uistiť, že po operácii môže určite zmeniť jej život. So svojimi prvotnými zisteniami to tak necíti. K samotnej operácii zatiaľ nezačali
